Registration For School Year 2023-2024 Is Happening Now! 

Forms and Documents Needed:

The following documents are required for enrollment of your child(ren):

• Student Enrollment Form SIS-10W 

• Form 14 Student Health Record - Physical examination, Tuberculosis clearance, Immunization record

• Verification of Birth:  State issued Birth Certificate or Passport

• Verification of Home Address:  Rental agreement/Rent receipt, Driver’s License, State ID (w/current address), current utility bill 

• Verification of Parent/Guardian:  Picture ID or Passport

• Legal Documents, if applicable: Adoption, Power of Attorney, Guardianship Letter, etc.

KINDERGARTEN REGISTRATION 

Any child who is born from August 1, 2017 to July 31, 2018 should enroll for Kindergarten. 

Children who are 5 years old by July 31 of any year are required by law to enroll in kindergarten for that school year. For example, children who are 5 years old by July 31, 2023 enroll in kindergarten for the 2023-24 school year. Children who turn 5 on August 1 or later enroll the following school year.
